MS ID Card

The Rocky Mountain Multiple Sclerosis Center offers MS ID cards as a service to those who live with with multiple sclerosis. The card lists vital information on one side and common MS symptoms on the back.

To receive an MS ID Card, please open and print the PDF form below.

Verification of your MS diagnosis is required along with a $10 processing fee. You may have your physician sign a statement of your diagnosis on medical office letterhead or a prescription pad. Physician’s verification may be faxed to 303-788-5418 or mailed to the address below. Please allow 10 to 12 business days for processing and delivery.
